Abstract

Recurrent thunderclap headache due to spinal subarachnoid hemorrhage (SAH) has been rarely reported. A 58-year-old woman presented with three episodes of thunderclap headache within a one-month period. Lumbar puncture revealed an SAH. After ten days from third episode of headache, the patient complained of back pain and worsened symptoms. Spinal MRI revealed spinal SAH. We suggest that spinal cord evaluation should be considered in patients with thunderclap headache and back pain without evidence of intracranial artery aneurysmal rupture.
